Special investigation of Infantile Paralysis, 1916.

These are the official data collected by the Health Department workers (some 30 nurses) who canvassed all neighborhoods in New York City in the summer of 1916, as a part of the epidemiological investigation to determine which children were actually stricken with polio, the possible conditions contributing to the disease, and whether a quarantine on the family was in force or necessary.
